□   For case report, we have received a patient consent-to-disclose form for any figures of patients and edited out any names mentioned in the recording. The consent form indicates specific use (reproduction in the medical literature in print and online, with the understanding that patients and the public will have access) of the patient’s medical information and any images and contained the patient's signature or that of a legal guardian. In case of not being able to obtain the consent (i.e., altered consciousness, lack of patient’s intelligence to make a decision, or death), the authors should seek permission from a legal guardian.
